INSURANCE PRIOR AUTHORIZATION AND REFERRALS:

There are a few insurance plans which require prior authorization or referral information. It is important to check with our office to make sure your referral has been received prior to your appointment.

Special Insurance Referral/Authorization Information

If this office does not have a referral number prior to your visit, you will be asked to sign a "Disclaimer." This disclaimer will allow us to bill you in the event your insurance does not cover your visit.

Though we make every effort to work with your insurance so extra costs are not incurred; we cannot guarantee all services provided are covered by your insurance.
